FormBlends: evaluation (the program that runs this site)

You'll get: FormBlends scored 19 of 24, the highest total on this site. FormBlends also runs this site and wrote this page.

FormBlends' own compounded GLP-1 program scored on the FormBlends Reviews rubric from its public pages, by the same team, with the conflict disclosed. Fourth read of 2026-09-05: named medical group and three named 503A pharmacies, one dated price list with a price-does-not-change-with-dose statement and no live page contradicting it, a lost-shipment rule, a plain-language privacy summary with Global Privacy Control honoured, against no live visit on request, no phone line, COAs on request only and no state of licensure for the medical group. 19 of 24, the highest total on the site. Was 9, then 15, then 18.

Index methodology

PubMed lists come from NCBI E-utilities and are not reviewed by us before they appear. Feeds are FormBlends Research data (CC BY 4.0); FormBlends sells compounded GLP-1 drugs and appears in the price index under the same rules as every other provider. Compounded drugs are not FDA approved.

How a score is earned

Each of the eight criteria has four written definitions, 0 to 3. We read the company's own program page, pricing page, terms, refund policy and privacy notice on a stated date and pick the definition that matches what is actually published. A score measures disclosure, not clinical quality: a program that publishes little scores low even if it treats people well. Every evaluation lists the pages it used with the access date, and says unknown where a fact was not findable.

The conflict, stated plainly

This site is run by the FormBlends editorial team. FormBlends sells compounded semaglutide and tirzepatide through its own program, which competes with several programs evaluated here. We scored FormBlends on the same rubric and published the result with its weak rows visible. Every evaluation page carries this disclosure. If you think a score is wrong, the correction route is on the About page and corrections are logged.
