Each score comes from the evaluation page for that provider, where the one-line reason is expanded with the page it came from and the date it was read. The level definitions are on the rubric page. If a company changes a page, the score changes at the next re-check, not before.

  • Rubric version 1.0, published 2026-09-04. Scores are editorial assessments from public pages, not user reviews, not ratings, not a recommendation.
  • Every provider was scored by the same editorial team, including FormBlends, which runs this site and sells a competing compounded GLP-1 program. That conflict is disclosed on every evaluation page and in the copied text.
  • A score describes what a company publishes, not the quality of its clinical care. A program can publish little and treat patients well; the rubric cannot see that.
  • Compounded semaglutide and tirzepatide are not FDA approved and are not interchangeable with Wegovy, Ozempic, Zepbound or Mounjaro. Brand-only programs score on the same rows.
  • Prices and terms change. The evaluation date on each column is the date the pages were read.

This calculator is an arithmetic aid for medication a licensed prescriber has already prescribed. It does not recommend a dose. Confirm any result with your prescriber or pharmacist before use.
