What FormBlends offers

The FormBlends GLP-1 Weight Loss Program: a free online assessment, review by an independent licensed provider, and, if prescribed, compounded medication dispensed by a licensed 503A pharmacy. The terms describe a transactional model rather than a membership; some payments are processed through FormScripts.com. FormBlends states that it does not itself provide care, prescribe or dispense (terms, how it works).

Medications: brand or compounded

Compounded only: semaglutide with B12 injection, tirzepatide with B12 injection, and liraglutide, with oral semaglutide drops and a sublingual product listed as coming soon. No brand GLP-1 is offered. The home page, both product pages, the regulatory-status page, the terms and the pharmacy-and-quality page all state that compounded products are not FDA approved. Compounded drugs are not interchangeable with Wegovy, Ozempic, Zepbound or Mounjaro.

Price as published (2026-09-04)

Inconsistent across pages. The products page shows semaglutide $99, tirzepatide $349 and liraglutide $199, labelled as intro or assessment pricing. The tirzepatide product page shows a $99 assessment and no medication price. The FormBlends cost report shows semaglutide from $99 intro and tirzepatide $349, lists the recurring price as not verified, and scores FormBlends' own disclosure at 20 out of 100. The cost calculator uses a $199 a month planning rate for semaglutide. A comparison article (updated April 1, 2026) says semaglutide from $149 a month and tirzepatide from $199 a month, including medication, consultations and shipping. The footer offers free shipping on orders over $150. Price by dose strength and lab costs are not stated anywhere we read. A buyer cannot work out month one and month four from these pages, and that is what the pricing row scores.

Prescriber model

An asynchronous intake reviewed by independent licensed providers (MD, DO, NP or PA) licensed in the patient's state. The consent lists messaging, video and phone as possible modalities, but no page describes how to request a live visit. No medical group is named on the provider-network page. Availability is described as "select US states" with no list.

Pharmacy sourcing statements

Licensed 503A compounding pharmacies working to USP 797 and 800, lab-tested, cold-chain shipped. No pharmacy is named. The pharmacy-and-quality page says patients may request COA, sterility, lot and facility documentation. The FormBlends-vs-Ro article, on the same site, says FDA-registered 503B facilities with third-party purity testing. Those two statements cannot both describe the same supply arrangement, and the sourcing row is scored on the lower reading until the site reconciles them.

Refund terms as published

Dispensed prescription medication is not returnable once shipped. Refunds for ineligibility follow terms shown at intake or checkout, which means the not-prescribed refund is not on a public page. Damaged shipments are handled through support with no stated window. There are no subscription terms because the model is transactional. The terms provide for arbitration under AAA rules and Delaware law with a 30-day opt-out.

Support and privacy

Email (support@formblends.com) and a web form, Monday to Friday 9 am to 6 pm Eastern, with an average response of about two hours in business hours. No phone. No described route for clinical as opposed to billing questions. The privacy policy (effective April 3, 2026) describes FormBlends as a HIPAA business associate, states no sale of personal or health data and no ad targeting on health data; a separate HIPAA notice (updated July 13, 2026) is published. The CCPA page states no sale but lists sharing with marketing partners, which sits uneasily with the privacy policy and is noted in the score. No LegitScript, NABP or state board references were found.

Regulatory record, primary documents only

We searched the FDA warning letters database, FDA press announcements, ftc.gov, sec.gov, state attorney general releases and federal court listings for FormBlends on 2026-09-04 and found no document. The FDA's March 3, 2026 press release names no recipients. Absence of a document is not evidence of anything except absence of a document.

What to ask them (we are including ourselves)

1. Which states do you serve? Name the medical group and the pharmacy for mine.
2. Is my medication made under 503A or 503B? Your product pages and your comparison article disagree.
3. What will I pay in month one and month four at my dose, and which of the four published prices is current?
4. If the provider does not prescribe, what is refunded, and why is that not on a public page?
5. Which laboratory tests each lot, and will you send the certificate of analysis for my lot without my asking?
6. How do I reach a clinician, as opposed to support, and how fast?
