What FormBlends offers

The FormBlends GLP-1 Weight Loss Program: a free online assessment, review by a clinician of an independent medical group, and, if prescribed, compounded medication dispensed by a state-licensed 503A pharmacy. The terms describe FormBlends as an informational and marketing platform operated by Form Blends LLC (Wyoming, formed March 31, 2026, per the about page). FormBlends states that it does not itself provide care, prescribe or dispense, and that the medical group and the dispensing pharmacies are separate from it (terms; pharmacy and quality). The terms now name Wyoming as the governing law and arbitration seat (they said Delaware on the first read).

Medications: brand or compounded

Compounded only: semaglutide with B12 injection and tirzepatide with B12 injection. Liraglutide, listed at $199 on the first read, is now marked "not currently available" on the products page and its own page, and the pricing page states it "is not in the checkout catalog or the pharmacy price book, so it cannot be purchased". Retatrutide and oral semaglutide drops are listed as coming soon. No brand GLP-1 is offered. The home page, both product pages, the pricing page, the pharmacy-and-quality page, the terms and the telehealthtelehealthCare delivered by video, phone or messaging rather than in person, held to the same standard of care. consent all state that compounded products are not FDA approved, are not generic versions of the brands, and are not interchangeable with Wegovy, Ozempic, Zepbound or Mounjaro.

Price as published (2026-09-05, second read)

The pricing page (prices "approved July 12, 2026", "last verified against the checkout code September 5, 2026") now separates a one-time price from a Subscribe & Save renewal price for every plan. Semaglutide: first month $99; monthly $190 one-time or $165 on Subscribe & Save; 3 months $399 or $374; 6 months $714 or $689; 12 months $1,188 or $1,163. Tirzepatide: first month $149; monthly $239 or $214; 3 months $627 or $602; 6 months $1,194 or $1,169; 12 months $2,148 or $2,123. The page states that "every plan is a one-time upfront charge for the plan length. Nothing renews unless you choose Subscribe & Save", that renewals "bill the plan price minus a flat $25 subscriber credit", what the price includes (provider review, medication, injection supplies, cold-chain shipping, no membership or consultation fee) and what it does not (care outside the program, labs arranged elsewhere). The same first-month, monthly and Subscribe & Save figures appear on the products page, both product pages and, since its September 5 update, the body of the FormBlends-vs-Ro article. The terms (section 8) say the pricing page price is the price charged at checkout. The month-two ambiguity from the first read is resolved. On the third read the pricing page, the pharmacy-and-quality page and the price block on both product pages add one sentence: "The price does not change with dose: it is the same at 2.5 mg or 15 mg. Your clinician sets the dose. Shipping is included; lab work is not." The pricing page's not-included list now also reads "Lab work is not included; shipping is".

The four items that held this row at 2 on the third read, re-read at about 21:47 UTC on 2026-09-05. The cost calculator's FormBlends card now reads "Planning Estimate FormBlends Compounded Semaglutide $190/mo First month $99" and the page contains no $199 figure. The FormBlends-vs-Ro article, read live (freshly rendered, not from cache) and in its stored body, now shows "Starting Price: $99 first month, then $190/month", "Monthly Price Range: First month $99 or $149; then $190 to $239 per month (plans $190 to $2,148)", and a provider chart row reading "FormBlends | 92 | First month $99 (semaglutide) or $149 (tirzepatide), then $190 or $239 per month, physician-led"; the same corrected row appears in the other FormBlends comparison bodies we sampled (form-blends-vs-ro-body, the 2026 full comparison, hims-vs-ro-vs-found) and the experience-hub FormBlends-vs-Ro bodies now say "$99 for the first month, then $190 per month ($165 with Subscribe & Save)". The FormBlends cash price index now carries two FormBlends rows (semaglutide month one $99, month four $190; tirzepatide $149 then $239, shipping included, labs not) sourced "formblends.com/pricing, verified 2026-09-05" and scores FormBlends 90 of 100, while its finding 3 still lists FormBlends among providers with price-language conflicts on still-indexed pages and its own note says older articles "are being corrected"; the JSON feed at /feeds/glp1-prices.json carries the same two rows. What remains: the shipping policy page still says "any applicable shipping costs will be disclosed during the relevant checkout", a hedge rather than a different price, against the pricing page's "shipping is included". We did not read all 4,501 rewritten article bodies; we read the ones we had cited plus the two experience-hub FormBlends-vs-Ro bodies.

Prescriber model

The pharmacy-and-quality page, the where-we-serve page, the about page and a "trust facts" panel on both product pages now carry the same block: "Prescriptions are written by independent, state-licensed clinicians (MD, DO, NP or PA) of Recess MD LLC, a medical group separate from Form Blends LLC; you consent to telehealth care from Recess MD LLC providers during intake." The block lists what the clinician reviews before prescribing (medical history, current medications and allergies, pregnancy status, weight and height for BMIBMIWeight divided by height squared; a rough screening number that the labels use to describe who the medicines were studied in., contraindicationscontraindicationA condition listed on the label under which the medicine should not be used. including medullary thyroid carcinomamedullary thyroid carcinomaA rare thyroid cancer arising from C cells; a personal or family history is a listed contraindication. or MEN 2MEN 2An inherited condition that raises the risk of medullary thyroid cancer; a listed contraindication for these medicines. history, pancreatitispancreatitisInflammation of the pancreas, felt as severe upper-abdominal pain; the labels say to stop the medicine if it is suspected. history and current GLP-1 use, identity verification, uploaded labs or records) and the follow-up cadence ("a check-in is requested before dose changes"; portal messaging at any time; a yearly primary care lab visit recommended). The live-visit sentence is: "Where clinically needed, or where your state requires a synchronous encounter, the clinician may ask for a video or phone visit before prescribing." That is a visit the clinician or the state can trigger. No page says a patient can ask for one.

Licensure and states

States: the where-we-serve page lists all 50 states, DC and Puerto Rico as either "dispensed to patients here" (36) or "check availability at assessment" (16), and says plainly that the count "comes from the last 165 shipped orders, not from a marketing map". The home page hero reads "dispensed in 36 states and territories" and both statistic blocks now read "36 States and territories served"; the 47 figure from the earlier reads is gone. Medical group: Recess MD LLC is named on four pages and in the linked consent, with no state of registration or licence reference for the group itself; the pages say the individual clinician's name and licence number appear on the prescription and visit summary, which is after payment. Pharmacies: three are named with a city and state (below). Verification route: state medical boards and state boards of pharmacy are linked (through formblendsdirectory.com and formblendscompounding.com, both FormBlends sites), and the pharmacy licence number is available from support with an order number.

Pharmacy sourcing statements

The pharmacy-and-quality page (last verified September 5, 2026) now gives each pharmacy a designation line. Greenwich RX (legal name Greenwich Health & Wellness LLC, Tomball, Texas): "503A compounding pharmacy", sterile compounding, states compliance with USP 795, 797 and 800, dispenses GLP-1 therapies. The DrugStore on East State (Athens, Ohio): "503A compounding pharmacy", dispenses GLP-1 therapies. Chemistry Rx (Folcroft, Pennsylvania): "503A compounding pharmacy and 503B outsourcing facility", ACHC and URAC accredited, dispenses other compounded prescriptions. The page says the facts are the pharmacies' own public statements checked on September 5, 2026. A new section, "What 503A means for your medication", states that the prescription is patient-specific and compounded after the clinician writes it, that it is prepared under USP 795, 797 and 800, that "a beyond-use datebeyond-use dateThe date after which a compounded preparation should not be used, set by the pharmacy that made it. is assigned per the USP 797 category and printed on the label", and that "batch potency and sterility testing is available as a certificate of analysiscertificate of analysisA lab report from the pharmacy or its testing partner stating what a batch contains and how pure it is. on request". It then distinguishes the brands: "Zepbound, Mounjaro, Wegovy and Ozempic are FDA-approved products made by Lilly and Novo Nordisk with FDA-reviewed manufacturing. Compounded semaglutide and compounded tirzepatide are not FDA approved, are not generic versions of those products, and are not interchangeable with them", with links to the FDA's human drug compounding page and DailyMed. The same sourcing and 503A block is repeated in the trust panel on both product pages, which also carry the not-FDA-approved statement. The FormBlends-vs-Ro article now says "compounded by state-licensed 503A pharmacies" (it said 503B facilities on the first read). The how-it-works page still says fulfilment "may involve a licensed pharmacy or FDA-registered outsourcing facility". On the fourth read, the article bodies we sampled that had described FormBlends' pharmacy as 503B now say 503A, except the FormBlends-vs-Hims-vs-Ro article, whose stored body still says "Their 503B pharmacy partnerships" and that FormBlends "offers both compounded and brand-name options", which it does not.

Testing and certificates of analysis

Unchanged in substance. The COA section describes potency and sterility, lot number, test dates and the laboratory, says the COA is issued per lot, and says a patient can email support with the order and lot number to have the lot's COA requested from the dispensing pharmacy and sent. No lot COA is published, no COA is stated to ship with the order, the testing laboratory is not identified on the site, and the source of the active ingredient is not described.

Refund terms as published

The returns page (last updated September 5, 2026) is now seven rules. Rule 1: full refund if the provider does not prescribe, "typically within 5 to 10 business days, depending on your bank", without the patient having to ask. Rule 2: dispensed medication cannot be returned or refunded. Rule 3: damaged, spoiled or incorrect shipments replaced free if reported within 7 days by email with a photo. Rule 4, new: "If tracking shows no delivery 5 days after the carrier's expected date, email support@formblends.com with your order number. We open a carrier trace. Once the carrier confirms the loss, or after 10 business days without a delivery scan, whichever comes first, we reship at no charge or refund the order, your choice." The same rule now appears on the shipping policy page, which also gives a ship time of 3 to 7 business days after approval and compounding, cold-chain packaging and tracking by email. Rule 5: cancel Subscribe & Save by emailing support before the next billing date shown in the confirmation email, with written confirmation; no phone call is needed. Rule 6: refund request format, hours, and the "Escalate" route after 2 business days. Rule 7: talk to support before a chargeback. The policy is linked from the pricing page ("Full terms are on the returns and refunds page") and from the trust panel on both product pages ("Refunds and cancellation"), and the terms (sections 6 and 10) repeat its main points.

Support and privacy

The contact page still opens with "There is no phone line". Billing, order, shipping and refund questions: support@formblends.com and a web form, monitored Monday to Friday 9 am to 6 pm Eastern, typical reply about two business hours inside those hours, next business day outside them. Clinical questions after purchase: the treating clinician through the patient portal; "support staff cannot answer clinical questions and will route you there". Urgent symptoms: 911 or urgent care; the pharmacy page adds that "the portal is not monitored for emergencies". New: a "Request a callback" option on the contact form ("we do not run a phone line, but you can ask support to call you"), which lands in the support inbox and is confirmed by email. An "Escalate" route after 2 business days is described.

The privacy policy (last updated September 5, 2026) now opens with a section headed "The short version" covering what is collected, who sees it, how long it is kept and how to ask for access or deletion. A new "Tracking and analytics choices" section names the tools the site loads (PostHog, Google Analytics 4, the Meta pixel, first-party attribution storage, embedded media) and states: "Global Privacy Control: if your browser sends a Global Privacy Control signal, we honor it. Google Analytics and advertising storage are set to denied, the Meta pixel is not loaded, and PostHog is opted out of capturing." Readers without a GPC signal can opt out of the same tools by emailing a "Privacy request". Retention is stated in the summary and section 9 (account and order records while the account is active and as tax, fraud-prevention and pharmacy record-keeping laws require; prescription records for the period state pharmacy law sets); deletion requests are answered within 30 days, with the records that cannot be deleted named. Recipients of health information are listed (the reviewing MD, DO, NP or PA, the dispensing 503A pharmacy, the payment processor, carriers, technology providers under confidentiality agreements); "we never sell your personal information or health information" and "we do not use health information for advertising targeting" are stated; the Meta pixel is disclosed as measuring page views and assessment starts. A separate HIPAA notice (last updated July 13, 2026) is published. The California notice still lists "marketing partners delivering communications" among recipients, which the privacy policy's list does not.

Regulatory record, primary documents only

We searched the FDA warning letters database, FDA press announcements, ftc.gov, sec.gov, state attorney general releases and federal court listings for FormBlends on 2026-09-04 and found no document. That search was not re-run for either 2026-09-05 re-read; the regulatory section does not move scores. Absence of a document is not evidence of anything except absence of a document.

Total: 19 of 24. 9 of 24 on 2026-09-04; 15, 18 and 18 on the first three reads of 2026-09-05. This is now the highest total on the site: Calibrate 18, WeightWatchers Clinic 17, Ro and NovoCare Pharmacy 16, Hims & Hers and Found 15, Noom Med 14, Ivim Health and LillyDirect 13, Mochi Health 12, Henry Meds 11. Totals are sums of eight separate disclosure judgements, not a ranking, and the comparison builder will not pick for you. The same rubric, the same reading list and the same lower-score rule were applied to this page as to every other, by the team that also writes formblends.com; the licensure row held at 2 above was held on the same reading that holds Ro at 2, and the pricing row moved to 3 only after the pages we had named as contradicting the price list were read again and found corrected. A high total here means FormBlends now publishes enough to check; it says nothing about the clinician you will get, the vial you will receive, or the ticket you will open.

What we would still like to see, row by row

1. Licensure. State Recess MD LLC's state of registration or a licence or NPI reference for the group, and publish a licensed-in list rather than a dispensed-to list.
2. Prescriber. One sentence saying that a patient in any state can request a video or phone visit before prescribing, and how.
3. Sourcing. Update the how-it-works page so it no longer says fulfilment may involve an FDA-registered outsourcing facility, or say when it does, and correct the FormBlends-vs-Hims-vs-Ro article's 503B and brand-name sentences.
4. Pricing. Bring the shipping policy page into line with the pricing page (it still says shipping costs are disclosed at checkout), and finish the article sweep so the cash price index no longer has to list FormBlends among providers with conflicting still-indexed pages. The calculator, chart row, vs-Ro tables and index rows asked for on the third read are now corrected.
5. Refunds. A stated commitment on FormBlends' side for refund issue (the page gives only the bank's 5 to 10 business days), and a shipping page that matches the pricing page on shipping cost.
6. Supply. Name the testing laboratory, describe the active ingredient source, and send the lot COA with the shipment rather than on request.
7. Support. A phone or live option with hours, and a described route for urgent clinical concerns beyond "call 911".
8. Privacy. A California notice whose recipient list matches the privacy policy.
