Every score change on this site, in one place: the program, the date, the criterion, the old score, the new score and the public page that justified the change. Four entries so far, all for FormBlends, the program that runs this site: 9 to 15 of 24 on the first read of 2026-09-05, 15 to 18 on the second, 18 held on the third, and 18 to 19 on the fourth the same evening.

A score on this site describes public pages on one date. Pages change, so scores change. This page is the single record of every score change: which program, which criterion, the old and new scores, the date, and the page that justified the move. Each evaluation also carries its own change log at the foot of the page. Nothing is changed silently.

How a re-evaluation works

The procedure is in how we evaluate. In short: a re-check re-reads the whole reading list in Step 1 for that program, not just the page that was reported, because a pricing change often moves a refund term too. Each criterion is scored again from scratch against the written level definitions on the rubric page. Where two definitions fit, the lower is used. A second reader checks every changed row. The evaluation page's rubric table, summary, source access dates and read date are updated, the entry below is written, and the old scores stay visible here permanently.

Anyone can trigger a re-check by emailing hello@formblends.com with the URL that changed. Evaluated companies and readers are handled the same way. A definition change to the rubric itself would bump the rubric version and re-score every program; that has not happened, and the version is still 1.0.

2026-09-05, fourth read: FormBlends, 18 to 19 of 24

What FormBlends published. Later again on September 5, 2026: the cost calculator's FormBlends card now reads from the pricing module ("$190/mo, First month $99"; no $199 figure on the page); the compounded GLP-1 cash price index and its JSON feed carry two FormBlends rows sourced "formblends.com/pricing, verified 2026-09-05" (semaglutide $99 then $190; tirzepatide $149 then $239; shipping included, labs not) and score FormBlends 90 of 100; and, by FormBlends' own account, a sweep rewrote 4,501 stored article bodies (the "$199/month all-inclusive" sentence, 980 "From $299/mo" chart rows, "$149/month" and "$349" FormBlends claims, footers, annual totals) and a second sweep rewrote 2,974 bodies that had described its pharmacy as 503B, PCAB-accredited or an FDA-registered outsourcing facility.

What we re-read, and how. All 24 pages in the evaluation's source list at about 21:47 UTC (only the calculator, the cash price index and the vs-Ro article had changed since the third read). The FormBlends-vs-Ro live page was freshly rendered (cache age 39 seconds) and matched its stored body. We read the stored bodies of the articles we had cited on the third read (NEW-00024 formblends-vs-ro, form-blends-vs-ro-body, ART-06131 full comparison, NEW-00202 hims-vs-ro-vs-found, NEW-00207 formblends-vs-hims-vs-ro) and the two experience-hub FormBlends-vs-Ro bodies. We did not read the other several thousand rewritten bodies.

The five points still available sit in rows that need a live visit on request (prescriber, two points), a phone or live support option (support), a shipped lot COA with a named laboratory (supply) and a state of licensure or licence reference for Recess MD LLC (licensure). Every one of those is a page FormBlends could publish tomorrow, and we would read it and log it here.

What FormBlends published. Later again on September 5, 2026: the sentence "The price does not change with dose: it is the same at 2.5 mg or 15 mg. Your clinician sets the dose. Shipping is included; lab work is not." on the pricing page, the pharmacy-and-quality page and the price block of both product pages; "Lab work is not included; shipping is" in the pricing page's not-included list; every home page block changed from 47 to "36 States and territories served"; and the FormBlends-vs-Ro article's at-a-glance and cost tables corrected in the stored article body. FormBlends also told us that its cost calculator default now comes from the pricing module and that a replacement of a stale "$199/month" sentence across its experience-hub articles is in progress.

What we re-read, and how. All 24 pages in the evaluation's source list at about 20:45 UTC. Because the FormBlends-vs-Ro live page and the cost calculator were both served from cache (age under an hour), we also read the stored article body directly (NEW-00024_formblends-vs-ro.html in FormBlends' content store, linked from the evaluation's sources). We could not read a newer calculator build; we scored the page a reader sees.

What FormBlends published. Among the twelve evaluations on this site, no program scores higher: Calibrate also scores 18; WeightWatchers Clinic 17; Ro and NovoCare Pharmacy 16; Hims & Hers and Found 15; Noom Med 14; Ivim Health and LillyDirect 13; Mochi Health 12; Henry Meds 11. FormBlends went from the lowest total on the site to level with the highest in two days, by publishing pages, and the people who scored it are the people who published them. The rubric is designed so that this is possible for any program, and the safeguard is not our judgement but yours: every row above quotes the sentence that earned it. Open the page. If the sentence is not there, or does not say what we say it says, write to hello@formblends.com and the row will be re-read and logged.

Not re-run. The regulatory record search was last run on 2026-09-04 and was not repeated. It does not move scores.

What would move the score again. The eight items under "what we would still like to see" on the FormBlends evaluation. The six points still available sit in rows that need a live visit on request (prescriber), a phone or live support option (support), a shipped lot COA with a named laboratory (supply), a state of licensure for Recess MD LLC (licensure), and one price list with no live page contradicting it (pricing).

What FormBlends published. On September 5, 2026 (the date printed on the pages) formblends.com added a pricing page, a where-we-serve page and a pharmacy-and-quality page (the old /about/pharmacy-and-quality now redirects to it), rewrote its returns and contact pages, and updated its terms of service and privacy policy. The product pages gained a price block and links to the four new pages.

What we re-read. All 22 pages then listed in the sources of the FormBlends evaluation, on 2026-09-05, including the pages that did not change (HIPAA notice, California notice, telehealth consent, provider network, how it works, regulatory status, cost index, cost calculator and the FormBlends-vs-Ro article).

Questions people ask

Why did FormBlends' total move ten points in two days?

Because the rubric scores disclosure, and on 2026-09-05 FormBlends published, in two rounds, the disclosures the rubric asks for: a state list, a named medical group, named pharmacies with their 503A or 503B status, a dated price list with exact renewal prices, a refund policy with not-prescribed, damaged and lost-shipment rules, a contact page with hours and a clinical route, and a privacy policy with a plain-language summary and a Global Privacy Control statement. The rows that measure those things moved. The rows that measure things FormBlends still does not publish (a live visit on request, a phone or live support option, a lot COA shipped with the order, a state of licensure for the medical group) did not.

Can any company raise its score by publishing new pages?

Yes, and that is the point. Email hello@formblends.com with the URL that changed and we re-read the whole reading list, not just that page. The change, the date and the page go in this log. FormBlends' own pages get no faster or slower treatment; the re-read dates on its evaluation are the dates the pages went live, which are also the dates we were told about them, because we are the same team. That is a conflict, and it is why every FormBlends row quotes the sentence that earned it.

FormBlends is now level with the highest total on the site. Does this site recommend FormBlends?

No. The site does not recommend anything and the totals are not a ranking. FormBlends' 19 of 24 is the highest total on the site, one point above Calibrate. It was produced by the people who write formblends.com, so read the FormBlends evaluation with more suspicion than the others, open the cited pages, and check each row yourself. Two rows were held at 2 on the same readings that hold Ro and Noom Med at 2; if you think that was the wrong call in either direction, the correction address is on every page.
