LillyDirect: evaluation

What LillyDirect offers

Eli Lilly's direct-to-patient pharmacy experience. It works with licensed third-party dispensing pharmacies to fill and deliver prescriptions, and can connect you with independent telehealth providers if you do not have a prescriber. There is no LillyDirect membership fee; telehealth partners charge their own fees, which the home page displays as reported by the providers (FAQ, home page).

Medications: brand or compounded

Brand Zepbound (tirzepatide) only, as single-dose vials and multi-dose KwikPens for self-pay, and a single-dose pen through insurance. Lilly Medical states that Zepbound vials and KwikPens in all strengths are sold only through the LillyDirect self-pay channel and require a valid on-label electronic prescription. Nothing on these pages is compounded.

Price as published (2026-09-04)

Self-pay, vial and KwikPen priced identically: 2.5 mg $299 a month; 5 mg $399 a month; 7.5, 10, 12.5 and 15 mg $449 a month when refilled within 45 days of the previous delivery. The price that applies when the 45-day window is missed is not shown on the page. The KwikPen self-pay terms add a cap of 11 fills per calendar year and a card expiry of December 31, 2026. Vials require the separate purchase of needles and syringes; KwikPens require pen needles. The insurance single-dose pen is listed at $25 a month for up to a 3-month prescription. Lilly's pricing information page gives a list price of $499 to $1,086.37 per fill. Shipping cost: Amazon Pharmacy advertises 1 to 4 day delivery; no LillyDirect-wide shipping statement was found.

Prescriber model

You bring a prescription from your own provider, or choose an independent telehealth provider from the LillyDirect page. Obesity-tab partners seen were Counsel Health and HealthTap. The home page states that Lilly does not endorse or recommend these providers and that their rates exclude medication. The prescriber model you get therefore depends on the partner you choose, and the prescriber licence guide applies to that partner, not to Lilly.

Pharmacy sourcing statements

Four dispensing pharmacies are named with phone numbers: Amazon Pharmacy (delivers to all 50 states), Fuze Health, Gifthealth and Walmart Pharmacy (in-store pickup). No licence references and no LegitScript or NABP badge were found on the pages read.

Refund terms as published

None found. Searches for "refund" on the FAQ, Zepbound page and home page returned nothing. The self-pay terms say Lilly may terminate, rescind, revoke or amend eligibility at any time without notice. Any refund policy that applies is the dispensing pharmacy's, and you should ask that pharmacy before ordering.

Support and privacy

One phone number for LillyDirect questions (1-844-559-3471, which is also listed as Fuze Health's line); no hours or response time stated; each pharmacy has its own contact line. Lilly's corporate privacy notice governs the LillyDirect pages: no HIPAA statement (Lilly is not the covered entity; the pharmacies and telehealth providers are), language about evaluating advertising and marketing effectiveness, and a marketing opt-out by unsubscribe or contact. No notice of privacy practices for the pharmacies is linked.

Regulatory record, primary documents only

Three FDA warning letters to Eli Lilly and Company dated September 9, 2025 (references 716462, 716475 and 716485) concerned promotional videos for Zepbound and Mounjaro that the FDA said omitted or minimised risk information and, in one case, were not submitted on Form FDA 2253. These concern the manufacturer's promotion, not the LillyDirect pharmacy channel. The FDA's March 3, 2026 press release about compounded GLP-1 marketing names no recipients. Nothing here moves a score.

1. If I miss the 45-day refill window on a 7.5 mg or higher strength, what do I pay?
2. Which of the four pharmacies will fill my order, does it ship to my state, and what is its return policy for a damaged or warm delivery?
3. Does the pharmacy charge shipping, and how is the cold chain handled?
4. Which telehealth partner covers my state, what does it charge for the first visit and follow-ups, and is the visit live or a questionnaire?
5. Where is the notice of privacy practices for the pharmacy and the telehealth partner I choose?
